'Angina' of the papillary muscle: an overlooked but reversible etiology of mitral regurgitation.

چکیده
INTRODUCTION In the absence of acute myocardial infarction (MI), ischemic papillary muscle dysfunction (PMD) is a rare complication of coronary artery disease. Dynamic PMD results in intermittent mitral regurgitation (MR). We present a successfully-treated case of recurrent pulmonary edema due to dynamic PMD secondary to chronic coronary heart disease involving the mitral apparatus. A pathophysiologic similarity could be drawn with intermittent angina pectoris (“angina equivalent”).

A rare complication of AMI is papillary muscle rupture (PMR), occurring in 1% of patients with acute myocardial infarction (AMI). Mitral regurgitation related to PMR occurs at a median of 1 day (ranging from 1-14 days) after onset of myocardial infarction. PMR tends to occur in those having a first time AMI and those without extensive collateral circulation. PMR involves posteromedial papillary...

Background: Cardiac resynchronization therapy (CRT) has proved as an efficient treatment in patients with end stage Heart failure. Previous studies showed the beneficial long term effects of CRT on the severity of mitral regurgitation (MR). The aim of this study was to evaluate acute effects of CRT on severity of MR. Methods: This study was a double blind randomized trial performed from 1387 t...
متن کاملDetection and estimation of the degree of mitral regurgitation by range-gated pulsed doppler echocardiography.
Sensitivity and specificity of detection of mitral regurgitation was assessed by range-gated Doppler echocardiography. The degree of mitral regurgitation was also estimated by the depth and width of the regurgitant jet detected with Doppler and compared with that assessed by left ventriculography. Of 47 patients with an adequate Doppler study, 24 had no mitral regurgitation by ventriculography....
متن کاملAcute severe mitral regurgitation: consideration of papillary muscle architecture
We present a case of an individual who presented with acute severe mitral regurgitation in the setting of an inferior ST elevation myocardial infarction. Both transthoracic and transesophageal echocardiography demonstrated a posteriorly directed eccentric jet of severe mitral regurgitation with flail anterior mitral valve leaflet attached presumably to the anterior papillary muscle. Intraoperat...
متن کاملSuccessful operation in an old survivor of anomalous origin of the left coronary artery from the pulmonary trunk (Bland-White-Garland syndrome).
A case of anomalous origin of the left coronary artery from the pulmonary trunk is reported. The patient, a 64-year-old woman, presented with a history of angina and cardiac failure. She was known to have had a mitral systolic murmur since school age. Echocardiography showed clinically significant mitral regurgitation and highly unusual extensive calcification of the mitral valve chordae, papil...
